How Do I Qualify for the FreeHouse Community Clinic?
Individuals may qualify to receive care at the FreeHouse Community Clinic
based on any of the following:

Participation in SNAP (Supplemental Nutrition Assistance Program)

Participation in a Supplemental Nutrition Program for Women, Infants & Children (WIC)
-
Recipient of Federal Public Housing Assistance
-
Participation in a Tribal Assistance Program
-
Medicaid Recipient
-
Household income 200% or less than the Federal Poverty Guidelines (see table below)
